Get histopathology requests signed.
Upload your histopathology request, pathology request form, or Portuguese requisição de histopatologia. Add the referring physician, lab, and any other parties who need to sign. Each gets a private link, and you get the completed form back with proof of every signature. Signers need no account.
The document is stored so your recipients can open it, reachable only by private link, and you can delete it any time.
5 free signature requests per month. Free account required to send.

Questions
Where do I start?
Head to www.penstead.com/histopathology-signature, upload your histopathology or pathology request form (requisição de histopatologia), add each party's name and email, and place signature fields for everyone. You get the signed form with a complete audit trail when all parties are done.
What file types work?
PDF, Word documents, and images. Most histopathology and pathology request forms are one of these. Images and scans convert to PDF on upload.
Is it free, and do signers need accounts?
A free account sends 5 signature requests a month. You need that account to send; the people signing need nothing. Pro is $15 a month for 50 requests.
Where is the request stored?
Editing runs in your browser. After you send, the document is stored so signers can open their links, behind an unguessable URL that is not listed or searchable. Signing links expire after 30 days; the completed request stays until you delete it.
What proof do I get that everyone signed?
Every completed document comes with a Certificate of Completion showing who signed, each person's email address, when they opened and signed the document, their IP address, and SHA-256 hashes of the original and signed files. The record is kept after completion.
